<h2>Cavernous Sinus Thrombosis</h2>
<p>Fewer than 50% of patients recover fully after being treated with antibiotics for septic thrombosis. In fact, thrombosis as a complication of infections of the paranasal sinuses is fatal in nearly 30% of patients. <h2>The Medical and Surgical Management of Sinusitis in Children</h2>
<p>As the number one chronic illness among all age groups in the US, sinusitis is a common experience among the pediatric age group. The factors leading to sinusitis are varied, and so diagnosis may be based on clinical manifestations or may need more intensive studies with x-rays or CT scans.</p>
<p>Whereas the majority of cases respond to conventional medical treatment, some do not. It is only then that surgical intervention is an option. Functional Endonasal Sinus Surgery is proving to be an effective treatment for chronic and recurrent cases of sinusitis.</p>

<h2>Article Abstract:</h2>
<p>Subperiosteal abscess is a serious orbital complication of sinusitis that occurs predominantly in children. Infection may spread to the orbital site through a preexisting defect in the lamina papyracea or by means of a thrombophlebitic process. Although the traditional approach to treating a subperiosteal abscess has been drainage through an external incision, we treated this serious complication with functional endonasal sinus surgery. Ten pediatric patients with clinical and CT evidence of orbital subperiosteal abscess underwent endoscopic surgery at LeBonheur Children&#8217;s Hospital in Memphis, Tenn., between 3 and 12 years of age at the time of surgery. Despite 2 to 4 days of intravenous antibiotic therapy, their signs and symptoms progressed, and axial and coronal CT scans demonstrated orbital subperiosteal abscess. Seven patients had bilateral ethmoid and maxillary sinusitis, and the remaining three patients had unilateral ethmoiditis. Opthalmologic assessments were performed for all patients immediately after hospital admission. The patients were started on regimens of high-dose intravenous cefuroxime sodium to subdue infections and pediatric decongestant nasal spray to facilitate drainage. All 10 patients with the confirmed diagnosis of subperiosteal abscess were treated with endoscopic surgery to drain the site of infection. All 10 patients improved significantly 12 to 24 hours after surgery and were discharged from the hospital to their homes 48 to 72 hours after surgery. Patients were continued on a regimen of broad-spectrum oral antibiotics and beclomethasone nasal spray for 2 weeks beyond the final examination. Follow-up endoscopic examinations were performed under general anesthesia in the operating room 2 to 3 weeks after surgery. No complications or adverse sequelae occurred. Our study appears to confirm that endoscopic drainage of subperiosteal abscess offers the patient an alternative to incisional surgery. The endoscopic approach prevents an external scar without increasing morbidity or length of hospitalization.</p>

<h2>Management of the Anomalous Pediatric Airway</h2>
<p>The pediatric airway is such a small passage that a change of just one millimeter can decrease a child&#8217;s airway by 40%. Dr. Rande Lazar, a pediatric specialist in Memphis, Tennessee, has researched anomalous pediatric airways for much of his career.</p>
<p>It has been established that the single most common cause of such changes is a condition known as Stridor. Dr. Lazar has determined that Stridor, caused by a turbulent airflow through the airway, is a symptom, not a diagnosis of what is wrong in the throat.</p>

<h2>Article Abstract:</h2>
<p>Toxic shock syndrome (TSS) is an uncommon, severe, multisystem illness that may follow any surgical procedure. It usually occurs in the immediate postoperative period and is manifested by the sudden onset of high fever and a variety of other signs and symptoms. The reported incidence of TSS after nasal surgery is 16 cases per 100,000 patients. We report five unusual cases of delayed TSS that occurred after functional endonasal sinus surgery in which no packing was used. Toxic shock syndrome developed in three children and two adults 5 days to 5 weeks postoperatively.  All patients were treated successfully with no sequelae. The pathophysiologic features, clinical manifestations, and treatment of TSS are described in detail.</p>

<h2>Abstract:</h2>
<p>A medial subperiosteal abscess (SPA) of the orbit is the most common serious complication of sinusitis in children. The distinction between SPA and the more benign pre-septal disease is difficult to make especially in a young child in whom an opthalmological evaluation is often difficult. Computerized tomography (CT) is the investigation of choice in making this distinction. Subperiosteal inflammatory disease of the orbit is initially treated with intravenous antibiotic therapy with surgery reserved for those patients who do not respond to medical treatment and in whom a medial SPA is confirmed by CT. Conventionally, the abscess is drained via an external incision and an ethmoidectomy is performed at the same time. More recently, successful drainage of SPA&#8217;s has been accomplished endoscopically via a intranasal approach with less morbidity and superior cosmesis. We present a 5 year experience of 24 patients with CT scans suggestive of medial SPA who underwent endoscopic exploration of the medial subperiosteal orbital space. We discuss the current management of medial subperiosteal disease of the orbit in children and include a review of the literature. Also included is a clinical staging system which aids the management of orbital complications of sinusitis.</p>

<h2>Article Abstract:</h2>
<p>The surgical closure of dry tympanic membrane perforations in children remains a controversial issue due to conflicting opinions on the appropriate technique, graft material and success rate. We present a review of 342 children who underwent fat graft myringoplasty as a day-stay procedure over a six-year period. Successful closure of the tympanic membrane perforation was achieved in 92 per cent of ears. Subsequent recurrent otitis media with effusion required insertion of ventilation tubes in 12 per cent. No relationship was observed between the age of the child and a successful outcome. We conclude that day-stay fat graft myringoplasty is a safe and successful procedure which results in a dry and safe ear in the majority of children.</p>

<h2>Article Abstract:</h2>
<p>Sinusitis is commonly seen in pediatrics. It is a dynamic, multifactorial process whose symptoms are variable. Upper respiratory tract infections and allergy are the major predisposing causes of sinusitis. Most patients respond to medical treatment. Surgery is generally indicated when optimal conservative management fails.</p>
<p>Functional endonasal sinus surgery (FESS) using endoscopes can form the mainstay of surgical therapy for pediatric patients with chronic/recurrent sinusitis. It is a surgical approach that can potentially restore the physiologic function of the paranasal sinuses. Although it was only introduced 5 years ago by Kennedy, it is becoming a popular surgical treatment for sinus disease in the United States. Most past reports have addressed its use in the adult population.</p>
<p>In children, we believe that FESS may be a curative and safe procedure. Gross et al. were the first to report on its use in the pediatric age group. They reported their experience with a series of 57 patients. The diagnosis of sinusitis in children may be difficult. The child is frequently unable to communicate many of the characteristic symptoms of sinusitis. Additionally, radiographic findings may be confusing. Ostensibly normal children may have abnormal findings on plain radiographs.</p>
<p>With the introduction of a wide variety of chemotherapeutic agents and new technologies, our therapeutic choices have become very broad. Accurate diagnosis and appropriate decisions can be made when a thorough understanding of the pathophysiologic mechanism of the disease entity is known.</p>
